TforT: Certificate in Co-Facilitation with a Patient/Family/Caregiver Partner

The Teaching for Transformation (TforT) Co-Facilitation Certificate program is offered in collaboration by the Centre for Advancing Collaborative Healthcare & Education (CACHE) and the Centre for Faculty Development (CFD).

Program Overview:

The Teaching for Transformation (TforT) Co-Facilitation Certificate Program is an interactive and immersive learning experience designed for individuals interested in co-facilitating interprofessional educational initiatives informed by humanistic and transformative education practices.

When we say co-facilitation, we mean collaborative facilitation by a patient/family/caregiver partner in partnership with a faculty member, learner, or practitioner. Participants will engage in hands-on practice, receive feedback, and build essential skills for leading and facilitating interprofessional education.

Participants who complete all components will receive a formal certificate of completion from CACHE & CFD.

  • Health and social care learners and practitioners interested in transformational teaching practices

Program Components (6.5 hours total)

  1. Preparatory Workshop: Monday, January 12, 2026 |  3:00–6:00pm EST |  Zoom Meetings (online)
    The preparatory workshop orients participants to the co-facilitation model, with a focus on the role of power relations and the importance of partnership in interprofessional education. Participants will engage with case material, reflect on their own experiences, and connect with their co-facilitator.
  2. Pre-Work – To be completed before January 19, 2026:
    • View a pre-recorded orientation video (11 minutes)
    • Review the Activity Guide & Reader’s Theatre Script
    • Meet with your co-facilitator partner
  3. Co-Facilitation in Action: Cultivating Team Partnerships: Monday, January 19, 2026 |  3:30–5:30pm EST |  University of Toronto St. George Campus (in person) OR Online via Zoom
    Students will explore partnership with patients/clients/families/caregivers in a team contexts by learning from a patient/family/caregiver partner and engaging with a Reader’s Theatre script on partnership developed by University of Toronto students and faculty.
  4. Co-Facilitation Debrief – Immediately following the activity (January 19, 2026)
    • Partner Debrief (15 minutes): Reflect with your co-facilitator on the experience.
    • Large Group Debrief (30 minutes): Convened with activity leads and fellow facilitators to share insights, challenges, and takeaways.
